When you earn a reward, you will get a Visa card in the mail. This card can be used anywhere that accepts Visa. You cannot use it for tobacco, alcohol or firearms. After you use your reward balance, do not throw away the card! This card will refill every time you complete a health action that qualifies for a reward.

If you want to check your reward balance, you can call the phone number on the back of your card and follow the prompts. You can also log in to the portal and create your account. You can check your balance and ask for a replacement card if you need to.
